Team Development Assessment Questionnaire1,2

This questionnaire is designed to help you identify the current stage of development of your team. Team members should complete the questionnaire individually. Then the scores for all team members should be added to identify the highest cumulative scores for relationships and for tasks.

Choose from each of the following 36 sets of statements the one that best describes the activities of your team at its current stage of development. Read the statements carefully and be sure to judge the present behavior, not the behavior as it existed in the past. Respond to each question by "clicking" on the answer that you think best describes your personal performance.
